Health-related forms, referral documents, insurance details, and clinical information should only be provided through approved instructions from Excel Physical Health & Wellness.

When you request a call, we collect only what we need to respond: your name, phone number, email address, preferred contact method, a general reason for contact, and a preferred callback window. The form does not include a field for health details, and we ask that you do not include any. We do not use advertising cookies, and analytics are disabled on this site. If analytics are added in the future, they will be configured in a privacy-protective manner and this policy will be updated first.

Your request is transmitted over an encrypted connection (HTTPS) and routed to our office, who use it to contact you and provide next steps. Access is limited to staff who need it to respond to you.
